Need help with xsl - from anyone

Well, I simply just don't know XSL and frankly, I just don't have the time to learn a new coding language. I'm muddling my way through PHP & SQL, and learning linux so my brain is on overload. I'm having a problem trying to customize our statements. I don't want dx codes (I can make that quit printing), I want it to pull the provider's name and address since I have more than one provider, I want it to put the payments on separate lines with description of the payment type (i.e. insurance, patient copay, patient pmt on account). I just can't get it done. If anyone could give me a little guidance, I would GREATLY appreciate it.
I would even be willing to just pull the same query in sql language if someone knows how to convert it to sql - then I could customize it because I somewhat understand and get it into a "report" - that much I can do.

You can just comment out or remove the blocks in the statement.xsl file in REMITT if you don't want something showing up. It takes standard HTML / XML comments.
